１型糖尿病是一種自體免疫性疾病，多發于兒童和青少年，患者自身免疫系統會錯誤攻擊并摧毀分泌胰島素的貝塔細胞。該病的一種治療方式是保護和恢復功能性貝塔細胞，但貝塔細胞替代療法注定無法成功，因為所注射的新細胞同樣受到免疫系統攻擊。
美國匹茲堡大學的研究人員通過一種新型的基因治療方法，將攜帶兩個重編碼基因Ｐｄｘ１和ＭａｆＡ的腺相關病毒載體通過胰導管，遞送至１型糖尿病小鼠的胰腺，能將阿爾法細胞轉化為分泌胰島素的功能性貝塔樣細胞。
發表在新一期美國《細胞－干細胞》期刊上的結果顯示，這一療法中生成的新生貝塔樣細胞與原生貝塔細胞有微小差別，對自身免疫攻擊相對有抵抗力。經單次治療后，１型糖尿病小鼠的血糖恢復至正常水平，并維持長達４個月。４個月后，小鼠又重新恢復高血糖，說明此基因療法對１型糖尿病尚無法*。
Type 1 diabetes is an autoimmune disease, mostly in children and adolescents. The patient's own immune system will attack and destroy beta cells that secrete insulin. The treatment of this disease is to protect and restore functional beta cells, but beta cell replacement therapy is bound to fail, because the new cells injected are also attacked by the immune system.
Researchers at the University of Pittsburgh through a novel method of gene therapy, will carry two heavy encoding genes Pdx1 and MafA recombinant adeno-associated virus vector through the pancreatic duct, delivered to the type 1 diabetic mice pancreas, the Alfa cells into functional insulin producing beta cells.
Published in the new issue of Journal of cell stem cell, we find that the new beta cells produced by this therapy are slightly different from native beta cells, and are relatively resistant to autoimmune attacks. After a single treatment, the blood sugar of type 1 diabetic mice recovered to the normal level and lasted for up to 4 months. 4 months later, the mice resumed their hyperglycemia, suggesting that the gene therapy could not cure type 1 diabetes.
